Lost or Stolen LA County Certification Card |

Card Replacement Process
If you have lost or damaged your Los Angeles County
scuba certification card, getting it replaced is a
simple process by completing the
Replacement Card Form and sending in payment.
You can send the form via email directly from the form
itself, or you can print it out and either fax or mail
it to us for processing.
Providing us with approximate dates of the course,
instructor name and certification card number greatly
expedites the process and helps us locate the original
course records that much faster.
There is a $25.00 fee per replacement certification
card. For your convenience you can pay online in
our card replacement section in our
eStore, or if you prefer you can mail your check
along with the completed card replacement form to:
